Thank you for visiting our web site. The Downtown Tempe Community office is located on the corner of 3rd Street and Mill Avenue, just 5 blocks from the ASU campus. If you need to get in touch with the DTC please feel free to write, fax, phone or e-mail us.

Maintenance Request Form

Together we can keep the Mill Avenue District clean, safe and inviting.  Please use this form to request cleaning services or report damage within the Mill Avenue District. Services available at no charge include:

  • Trash/ debris removal
  • Sidewalk Pressure Washing
  • Graffiti Removal
  • Empty public trash cans
  • Repair damaged street banners
  • Address damage/safety concerns
